GP Toolkit

The GP Toolkit is a web-based, information system which supports Practice Based Commissioners and Primary Care Trusts to identify opportunities to move money and resources around the health system to improve services, both inside and outside of hospital.

The GP Toolkit is a joint development with West Midlands Strategic Health Authority and Teamwork and is available under licence.

The current release of the GP Toolkit covers admitted care and is based on a 'best practice' template which identifies where services need to be improved - avoiding admissions, earlier discharge from hospital and improved patient management.
